 🦶ปัญหาภาวะเท้าตก (Foot Drop) : คือลักษณะที่กระดกปลายเท้าไม่ขึ้น ทำให้ตอนเดินจะเดินลากเท้า หรือก้าวเดินโดยงอข้อสะโพกและข้อเข่าเพิ่มขึ้น (Steppage gait) เพื่อยกปลายเท้าให้พ้นพื้น

👣สาเหตุ : ค่อนข้างหลากหลายทั้งจากการบาดเจ็บของเส้นประสาท หรือจากกล้ามเนื้อและเส้นประสาทผิดปกติ เช่นในกลุ่มผู้ป่วยความผิดปกติของระบบประสาทส่วนกลาง 🧠 โรคหลอดเลือดสมอง โรคเนื้องอกบริเวณสมองและไขสันหลัง และโรคกระดูกสันหลังทับเส้นประสาทไขสันหลัง หรือ โรคที่เกิดจากความผิดปกติของระบบประสาทส่วนปลาย 🦵โรคปลายประสาทอักเสบ

✨เบรสดามขากันเท้าตกมีชื่อเรียกหลากหลาย เช่น AFO (ankle foot orthosis)

✨AFOชนิดสำเร็จรูปเหล่านี้ ไม่เหมาะกับ การนำมาใช้ในผู้ที่มีข้อติด ผิดรูป หรือมีอาการเกร็งของกล้ามเนื้อมาก ทั้งนี้มักพิจารณาให้ใช้ชนิดหล่อทำเฉพาะบุคคลแทน

✨การพิจารณาชนิด รวมถึงดีไซน์ของ เบรสดามขาชนิดกันเท้าตกที่เหมาะสมในแต่ละบุคคลให้ปรึกษานักกายอุปกรณ์ (certified prosthetist&orthotist)

✨AFO ชนิดสำเร็จรูปมีข้อดี คือ มีให้เลือกหลากหลาย มักทดลองใส่ก่อนได้ ไม่ต้องรอผลิตของ ราคาถูกกว่าชนิดหล่อทำเฉพาะบุคคลมาก(เมื่อเทียบจากวัสดุแบบเดียวกัน)

✨โดยปกติเราจะต้องการเบรสดามขากันเท้าตกที่สามารถประคองข้อเท้าขึ้นมาได้ถึงท่ากลาง neutral position.(มองง่ายๆคือเท้ากับขาตั้งฉากกัน)

🦶ชนิดนี้มักมีแรงในการยกปลายเท้าขึ้น เพื่อป้องกันเท้าตกได้ค่อนข้างน้อย เมื่อเทียบกับชนิดอื่นๆ

🚶‍♂️ข้อดี : มักมีน้ำหนักเบา มีรูปแบบหลากหลายทั้งสายสลิงแบบสั้น/แบบยาว มีทั้งแบบที่รัดจากหลังเท้าแล้วพยุงขึ้นโดยตรงหรือแบบที่ยึดกับรองเท้า

💰ราคา: 2xx-2,xxx บาท (ขึ้นอยู่กับยี่ห้อ วัสดุ ดีไซน์)

📌ข้อเสนอแนะ: ส่วนตัวไม่แนะนำชนิดนี้สำหรับเป็นอุปกรณ์หลักในผู้ที่มีปัญหาเท้าตก เนื่องจากมีแรงประคองน้อย แต่หากผู้ใช้มีความต้องการจะใช้จริงๆ จะแนะนำให้ใช้เป็นตัวสำรอง สำหรับเดินในบ้านหรือระยะทางสั้นๆ

📍ปล. บางคนเห็นว่าหน้าตาคล้ายกันจนไปสับสนกับ ankle supportอื่นๆ เช่น ankle lace up brace วัตถุประสงค์ในการใช้งานต่างกันนะคะ ใช้แทนกันไม่ได้ ถ้าเป็นตัว ankle lace up brace พวกนี้ใช้ค่อนข้างได้ผลดีกับผู้ที่มีปัญหาข้อเท้าแพลง ไว้จะแนะนำในโพสต่อๆไป

🦶AFO ชนิดสำเร็จรูปแบบพลาสติก เป็นชนิดที่นิยมใช้กันมากทั่วโลก ให้แรงประคองที่เพียงพอในการประคองเท้าให้อยู่ในท่ากลาง ช่วยเรื่องปัญหาเดินเท้าตกได้ดี

🚶‍♂️ข้อดี : ราคาไม่สูง ปรับแก้ได้ง่าย ด้วยปืนร้อน

💰ราคา : 1,xxx-4,xxx บาท (ขึ้นอยู่กับยี่ห้อ ชนิดพลาสติก บริการหลังการขาย )

📌ข้อเสนอแนะ

✨หลักๆแล้วดีไซน์ภายนอกจะมี 2 แบบ คือแบบพลาสติกยาวเต็มเท้า กับแบบพลาสติกยาว ¾ ของความยาวเท้า ทั้งสองแบบนี้ไม่มีแบบไหนดีกว่ากันขึ้นอยู่กับการพิจารณาเฉพาะราย เช่นแบบเต็มเท้าจะยาวเต็มพื้นที่ในรองเท้าทำให้ไม่ขยับเลื่อนภายใน หรือแบบ3/4 จะสวมเข้าไปในรองเท้าง่ายกว่า (อย่างไรก็ตามยังมีปัจจัยอื่นๆให้นักกายอุปกรณ์พิจารณาร่วมด้วย)

✨พบว่าสินค้าพลาสติก AFO ในแต่ละยี่ห้อ แม้หน้าตาภายนอกคล้ายๆกัน แต่มีรายละเอียดของทรงฝ่าเท้าไม่เหมือนกัน ความแข็ง/ความยืดหยุ่นของวัสดุก็ต่างกัน

✨วิธีหา AFO ที่เหมาะสมกับตนเอง คุณควรได้รับการประเมินจากนักกายอุปกรณ์ เพื่อคัดเลือก AFO ที่เหมาะสมที่สุด

✨ส่วนตัวไม่พบว่ามี AFO ตัวไหนที่จะดีที่สุดสำหรับผู้ใช้ทุกๆคน เช่น บางคนใช้ยี่ห้อ OTS Grenace AFO แล้วเดินได้ดี ขณะที่บางคนใช้ Supralite AFO แล้วกลับเดินได้คล่องกว่า ทั้งนี้มีทั้งปัจจัยทั้งจากตัวผู้ใช้เอง เช่น อายุ น้ำหนัก อาการของโรค 👉 อ่อนแรงมาก หรือพอมีกำลังบ้าง หรือมีอาการเกร็งร่วมด้วยหรือไม่ ฯลฯ) ร่วมกับปัจจัยความแข็ง, ความยืดหยุ่น, ดีไซน์ ของพลาสติก AFO ร่วมกันทำให้ผลออกมาแตกต่าง

✨เนื่องจากเป็นอุปกรณ์สำเร็จรูป ดังนั้นอาจใส่ได้ไม่พอดีเป๊ะ หากมีปัญหาผู้ใช้ไม่ควรปรับแก้ ตัด ดัดด้วยตัวเอง เดี๋ยวจะยิ่งพัง ควรปรึกษานักกายอุปกรณ์

✨ต้องใส่กับรองเท้าเท่านั้น ห้ามใส่แล้วเดินเท้าเปล่า มันจะลื่นล้ม

AFO ชนิดนี้วัสดุจะเป็นคาร์บอนไฟเบอร์ ตัวดีไซน์มีหลากหลายไม่แพ้แบบพลาสติก มีความแข็งแรงสูง ด้วยตัววัสดุทำให้มี energy return เมื่อก้าวเดิน ช่วยให้เดินคล่อง เดินง่าย ประหยัดพลังงานในการเดิน

ข้อแนะนำ

จากประสบการณ์พบว่าผู้ใช้ส่วนใหญ่กว่า 70 % (ตปท) ที่เคยใช้ plastic AFO พอใจที่จะเปลี่ยนมาใช้ carbon fibre AFO หลังได้ทดลองใช้ หลักๆคือตัวคุณสมบัติของวัสดุที่ช่วยให้เดินได้คล่องตัวกว่า, อีก 20% รู้สึกเฉยๆ ไม่แตกต่าง จึงเลือกที่จะใช้แบบพลาสติกเพราะถูกกว่าม และอีก 10% รู้สึกว่าแบบพลาสติกเดินง่ายกว่า (อย่างที่เคยกล่าวไปก่อนหน้า “ไม่พบว่ามี AFO ตัวไหนที่จะดีที่สุดสำหรับผู้ใช้ทุกๆคน”)
ในไทยตัว carbon fibre AFO ยังไม่นิยมมากนัก ส่วนหนึ่ง ด้วยราคาที่ค่อนข้างสูง

TurboMed XTERN AFO ตัวนี้เป็น AFO ดีไซน์ใหม่ที่น่าสนใจดีค่ะ เลยเอามาใส่ไว้ด้วย มองว่ามีข้อดีไม่แพ้ ชนิดที่ ๓ carbon fibre AFO ในแง่ของการช่วยให้เดินง่าย เดินคล่องขึ้น ด้วยดีไซน์ที่ยึดกับรองเท้าโดยตรงทำให้ไม่ไปเกะกะผิวผู้ใช้งาน ผู้ใช้บางคนที่เคยใช้ carbon fibre AFO จึงเลือกเปลี่ยนมาใช้TurboMed XTERN AFO

ข้อแนะนำ :

จากประสบการณ์การใช้ตัวนี้ใน ตปท พบว่าผู้ใช้เดิมยังเลือก carbon fibre AFO อยู่ เพราะชอบดีไซน์แบบเดิมๆ และราคาที่ถูกกว่า (ที่ ตปท ราคา carbon fibre AFO เพียงหมื่นต้นๆ)

รองเท้าสำหรับปัญหาเท้า

รองเท้าคนพิการ คืออะไร 

  

 คำว่า " รองเท้าคนพิการ" เป็นคำเรียกที่ให้เข้าใจได้ง่ายว่าเป็นรองเท้าตัดพิเศษสำหรับผู้มีเท้าพิการผิดรูปหรือมีปัญหาเท้าอื่นๆ

การที่เป็นรองเท้าตัดพิเศษนี้ หมายถึงเป็นการถูกผลิตโดยทำจากสัดส่วนเท้าเฉพาะบุคคล หรือเรียกว่ารองเท้าตัดเฉพาะบุคคล หรือ custom made shoe 

รองเท้าตัดเฉพาะบุคคล ไม่ได้จำเพาะว่าต้องเป็นรองเท้าคนพิการ จะเป็นรองเท้าของผู้มีเท้าปกติก็ได้ มีร้านทำรองเท้าหลายร้านในไทยที่เปิดรับตัดรองเท้าเฉพาะบุคคลสำหรับรูปเท้าปกติ 

                 รองเท้ากลุ่มนี้เป็นตัวเลือกที่ดีในการใช้แก้ปัญหาของเท้า ในไทยถือว่ามีโรงพยาบาลรัฐหลายแห่งที่มีคลินิกกายอุปกรณ์ที่มีศักยภาพในการตัดรองเท้าสำหรับผู้พิการได้ อย่างไรก็ตามรองเท้ากลุ่มนี้ยังมีตัวเลือกให้เข้าถึงน้อยกว่า ราคาสูงกว่ารองเท้าสำเร็จรูปตามท้องตลาด รวมถึงปัญหาระยะเวลาในการผลิต

ผู้ที่มีปัญหาเท้าผิดรูปหรือมีปัญหาเท้าอื่นๆ จำเป็นต้องใช้รองเท้าคนพิการเสมอไปหรือไม่ 

       - คำตอบคือไม่เสมอไป 

         รองเท้าที่มีจำหน่ายตามท้องตลาดมีความหลากหลายให้เลือกมากมาย ทั้งดีไซน์ รูปแบบ วัสดุ ราคา มีทั้งที่เป็นรองเท้าแฟชั่น รองเท้าสุขภาพ รองเท้าวิ่ง และอื่นๆ 

           หลายรายที่ผู้มีปัญหาที่เท้าสามารถใช้รองเท้าที่มีจำหน่ายตามท้องตลาด(โดยเลือกรูปแบบที่เหมาะสม)ร่วมกับกายอุปกรณ์เฉพาะบุคคล( orthotics)ได้ดี โดยไม่จำเป็นต้องใช้รองเท้าตัดเฉพาะบุคคล

            หลายรายที่การดัดแปลงรองเท้าปกติ(Shoe modification) นั้นสามารถนำมาใช้กับเท้าที่ผิดรูปหรือมีปัญหาได้ โดยไม่จำเป็นต้องใช้รองเท้าตัดเฉพาะบุคคล

            แต่แน่นอนค่ะว่ามีบางรายที่ใช้วิธีข้างต้นทั้งสองไม่ได้ ก็จะนำมาสู่ตัวเลือกที่สาม คือ ตัดรองเท้าเฉพาะบุคคล

ตัวอย่างเคส ตัดรองเท้าเฉพาะบุคคล

          นาย ก อายุ 60 ปี มีปัญหาเท้าผิดรูปตั้งแต่เด็ก ไม่เคยได้รับการรักษา ไม่เคยใช้รองเท้าพิเศษใดๆ ปกติจะใช้รองเท้าแตะธรรมดาหรือไม่ใช้เลยเพราะรองเท้าเกาะเท้าได้ยาก เท้าขวาเกือบปกติมีการผิดรูปแบบบิดเข้าในเล็กน้อย เท้าซ้ายติดในท่าเขย่งไม่สามารถดัดกลับได้ นิ้วเท้าซ้ายติดในท่าแบะออก

           - เมื่ออายุมากขึ้นปัญหาจากความผิดรูปของเท้าเริ่มชัดขึ้น ความมั่นคงในการเดินลดลง เดินได้เพียงระยะสั้นๆกับ walking frame มีปัญหาแผลที่เท้า ปัญหาความเจ็บปวดที่เท้า

เคสนี้ใช้วิธีสแกนเท้าด้วย Isense 3D scanner ค่ะ หลังจากออกแบบรองเท้า เลือกวัสดุเรียบร้อย ก็ e -mail ไฟล์ไปให้ช่างทำรองเท้า ที่ฟิลิปปินส์ ส่วนตัวชอบวิธีนี้ค่ะ สะดวก ทุ่นแรงดี

เคสนี้ถามว่าใส่รองเท้าตัดเฉพาะบุคคลแล้วเดินง่ายเลยมั๊ย ก็ไม่นะคะ สังเกตมั๊ยคะว่ารองเท้าขวาถูกเติมพื้นให้หนาขึ้นมาเพื่อว่าจะทำให้ความสูงของขาทั้งสองข้างมันอยู่ในระดับเดียวกัน (คือให้ขายาวเท่ากัน) แต่ในรายที่เดินด้วยขาข้างนึงสั้นข้างนึงยาวต่อเนื่องมาหลายปีจนเคยชินอย่างนี้ จู่ๆจะให้เดินด้วยความสูงขาที่เท่ากัน เค้าจะปรับตัวไม่ทันแต่จะรู้สึกว่าการยืน/เดินมันไม่บาลานซ์เอามากๆๆ ความสูงของพื้นรองเท้าฝั่งขวาจึงต้องค่อยๆปรับเอาค่ะ เช่นในกรณ๊นี้ตัวพื้นรองเท้าฝั่งขวาก็จะถูกตัดออกประมาณครึ่งนึงก่อน เพื่อให้ผู้ใส่รู้สึกมั่นคงในการเดิน
